Forms
The following forms are used throughout your graduate program. If you are unsure which form you need or where to submit it, please contact the Departmental and Academic Administrator.
- Academic Change Form — Use this form to request changes to your academic registration or program details.
- Annual Progress Report — Required yearly submission documenting your research progress in consultation with your supervisor and supervisory committee.
- Canadian Common CV (CCV) — Required for most Canadian funding agency applications including NSERC and CIHR.
SGSPA Forms
The School of Graduate Studies and Postdoctoral Affairs (SGSPA) provides a number of forms for graduate students. Visit the SGSPA Forms page for the full list of all available forms. Students who require a PDF version of any form can email sgspa.reception@queensu.ca.
The most commonly used forms for DPMM students include:
- Award acceptance — required when accepting external awards such as OGS, CGS-M, OSOTF, and NSERC/SSHRC funding, as well as annual progress reports for NSERC and SSHRC award holders
- Thesis submission — including the Thesis Submission Form, Permission of Co-Authors form, and PhD Oral Examination Form, required as you approach degree completion
- Time Limit Extension — required to continue your studies beyond your expected completion period
Accommodations Portal
Ventus Portal - Ventus is the online portal that securely connects students, Queen's Student Accessibility Services (QSAS), the Exams Office, instructors, and other support services to manage and implement academic accommodations.
Student Support
Graduate Peer Support Centre
The Graduate Peer Support Centre offers free, confidential peer-based support to graduate and professional students at Queen's University. Volunteer peer supporters are trained in active listening, crisis intervention, and campus resource referral, providing a welcoming space for students who need someone to talk to.
The Centre is located in JDUC Room 350 and operates as a drop-in service. For current hours and virtual options, visit the Graduate Peer Support Centre website.
